Notary Verification of Identity Form

This form is used to verify your identity when you are unable to appear in person at the UMD Office of Student Financial Aid.

Parent Non-Filer/Low-Income Form

This form is used to verify that your parent did not file a U.S. income tax return and to collect information on what sources of income were used to cover the cost of living expenses.

Student Non-Filer/Low-Income Form

This form is used to verify that you (and your spouse, if applicable) did not file a U.S. income tax return and to collect information on what sources of income you had to cover your cost of living expenses.

Study Abroad Financial Aid Worksheet

You may apply your standard financial aid offer toward approved study abroad programs. This Permission to Study Abroad Form (PSA) must be completed prior to your study abroad experience(s).

Permission to Enroll Financial Aid Consortium Agreement

Students can use the Permission to Enroll process to request approval to take a course at a different school (Host Institution) and transfer the credits back to UMD towards their degree requirements. If you have an approved PTE and want to receive financial aid based on your combined enrollment at UMD and the Host Institution, you must submit a Permission to Enroll (PTE) Financial Aid Consortium Agreement.

FWS Appeal Form

Complete this form, if:

you did not receive FWS in your financial aid package and would like to request an offer, or
you have earned at least 66% of your current offer and would like to request an increase to continue working.
